Health Storylines™ is your personal self-care tool and health journal that lets you use the power of your story to personalize your care, create your own support network, and influence the way medicine cares for people like you in the future. It allows you to manage, track, and share anything related to your health journey and daily living, which gives you insights on your well-being.

Self Care Catalysts is a patient solutions, intelligence and analytics company that enables healthcare innovation. We are committed to advocating for patients and consumers when it comes to healthcare decisions. Our belief is that when patients are informed, respected, and engaged, they make better choices. Better choices mean better health outcomes.

Our team is based in Toronto and San Francisco, and is made up of a group of health researchers, scientists, doctors, healthcare experience designers and software developers with a desire to improve patient self-care.

Included on Health Storylines are a range of Health Tools to help you better manage your health condition(s). These Health Tools include a medication reminder, a symptom tracker, a daily journal, and more.

You can also personalize your Health Storylines according to your health condition, to receive additional Health Tools.

Health Storylines is intended for patients with chronic illnesses and for their caregivers, using it on the behalf of the patient.

That being said, anyone can use Health Storylines to better manage their health with a range of health tools.

Yes, Health Storylines is 100% free to download and to use. You do not have to pay to sign up for an account, and there is also no charge to continue using it.

Privacy and Security

Health Storylines collects the personal data you enter during the registration process, and your self-reported health data. Personal data includes your name and email address. Your personal data can be used to identify who you are, so we never share your personal data with anyone, and we treat your privacy very seriously.

Your self-reported health data entered on the app is aggregated to generate research insights that are shared with our partners and other third parties. It is impossible to identify your personal identity after the self-reported health data you enter is aggregated.

For example, we only share information such as “86% of cancer patients track their symptoms using Health Storylines”, and there is no personally-identifiable information attached to it. We never share whom the data is coming from.

We take reasonable steps to protect your personal information and self-reported health data from unauthorized access, use, disclosure or loss. Our platform has reasonable security measures (including online and offline physical, electronic and managerial safeguards) in place to protect against the loss, misuse and alteration of your data under our control. We use industry-standard encryption technologies to protect the data that you submit on our platform.

However, as with any transmission over the internet, there is always some element of risk in submitting personal information online and we will not be liable for any breach of data security caused by circumstances outside its control, including unauthorized network intrusions.

If you have lost your phone, do no worry because there are many ways to locate your phone.

To keep your information safe, it is recommended that you go onto Health Storylines on a different device (perhaps on a computer), log in, and change your password.

Listed here are some possible solutions to help you locate your phone:

On Android devices:

If you have location services turned on:

  1. Go to www.gmail.com, and log on to the account you have set up on your phone
  2. Go to www.google.com, and google “Find my phone”
  3. A map should show an accurate location of where your phone is located

If you do not have location services turned on:

  1. Go to www.gmail.com, and log on to the account you have set up on your phone
  2. Go to www.google.com, and google “Find my phone”
  3. Click on the map that appears
  4. Click on any of the options which you feel fits the situation

On your iPhone:

  1. Go to www.icloud.com/#find, and log on to the account you have set up on your phone
  2. iCloud will show a map after trying to locate your phone

HIPAA is the federal Health Insurance Portability and Accountability Act of 1996. The primary goal of the law is to make it easier for people to keep health insurance, protect the confidentiality and security of healthcare information and help the healthcare industry control administrative costs.

Yes, the Health Storylines platform is HIPAA compliant. All data reports that are generated from your self-reported health data entered onto Health Storylines are aggregated, de-identified data. De-identification follows HIPAA safe harbor guidelines.

Downloading the Health Storylines mobile app

Health Storylines is available on the web and also on your mobile device.

To access Health Storylines on the web:

  1. Visit healthstorylines.com/app/
  2. Click on “Sign Up”. You will need to enter in the registration information and agree to the Privacy Policy and Terms of Use to begin using Health Storylines.
  3. Now you can start using the web version of Health Storylines

To get Health Storylines on your Android device:

  1. Go on the Play Store™ on your Android device, and search for “Health Storylines”
  2. Click the INSTALL button
  3. You will be prompted with APP permission screen, click ACCEPT
  4. Once installed, click on the Health Storylines app
  5. Click on “Sign Up”. You will need to enter in the registration information and agree to the Privacy Policy and Terms of Use to begin using Health Storylines.
  6. Now you can start using your Android version of Health Storylines

To get Health Storylines on your iPhone or iPad:

  1. Go on the App Store on your iPhone or iPad, and search for “Health Storylines”
  2. Click the GET button and then click INSTALL
  3. Once installed, click on the Health Storylines app
  4. Click on “Sign Up”. You will need to enter in the registration information and agree to the Privacy Policy and Terms of Use to begin using Health Storylines.
  5. Now you can start using your Android version of Health Storylines

Health Storylines is free to download and to use, and you never have to pay for it. However, sometimes the Apple iTunes Store does ask for your payment information, and this is not associated with the use of Health Storylines.

If you are creating a new Apple ID, you can choose the option of not inputting your credit card information. There should be a “none” option under “Payment Type”. If you already have an Apple ID, iTunes Store requires you to input your credit card information, but don’t worry, you can remove this information right after.

To remove this information, go onto the Home Screen and select Settings. Go to iTunes & App Store. Select your Apple ID then choose View Apple ID. Select Payment Type and choose None.

If you use Health Storylines over a Wi-Fi network, no additional mobile data plan usage will occur. Using it over mobile data network may result in small increase in data usage. At this point in time, our app is based on web technology. As such, the app will use as much data as using a small lightweight website that does not have a lot of media that takes up data.

Signing Up, Logging in & Customizing Your Profile

To sign up for an account, you need to enter your first name, a valid email address, create a password, your gender, and year of birth. We also require you to agree to our privacy policy and terms of use.

If you cannot log in, do not panic. Check to see if you have entered the right email or password. If you have forgotten your password, click on the “Forgot Password” link on the login page and then enter your email address on the next page. You will receive an email to reset your password.

  3. Click on “Click to Change Password”
  4. Type in your new password in “New Password” and “Confirm Password”
  5. Click on “Update Profile”

  3. Type in your email
  4. Click “Submit”
  5. Follow the steps from the link provided in the email that will be sent to you

  3. Type in your condition in the Condition section
  4. Click on the condition from the drop down menu that matches
  5. Click on the drop down menu to pick the year of diagnosis
  6. Click on “Add”
  7. Click on “Update Profile”

You can only remove a condition if you have previously added a condition

  1. Open the app on the device you are using and log in
  2. Click on “My Profile” located on the left side bar which can be seen when pressing the menu button on mobile or tablet devices
  3. Click on the X beside the condition you wish to remove
  4. Click on “OK” on the pop up that appears
  5. Click on “Update Profile”

By adding a condition, new Health Tools may be unlocked and added for you in order to better assist you in your daily life as well as giving better insight in your own well-being.

Using the Health Tools on Health Storylines

With the Medication Tracker, you can add multiple medications to your list, and set reminders so you don’t forget to take them. You can track whether you have taken or missed your medications each day, and even the reasons why.

  1. Open the app on the device you are using and log in
  2. Click on the “Medication Tracker” Health Tool
  3. Click on “Add Medication”
  4. Type in the medication
  5. Click on the medication from the drop down menu that matches
  6. Add notes if needed and click “Next”
  7. Add reminder by clicking on “Add Reminder”
  8. Choose a time from the drop down menu
  9. Click “Finish” to add the medication

You can only remove a medication if you have previously added a medication.

  1. Open the app on the device you are using and log in
  2. Click on the “Medication Tracker” Health Tool
  3. Click on “Update medication”
  4. Click “Remove”
  5. Click “Save”

Emotional health is important to your overall health. With the Daily Moods health tool, you can track your mood daily from a collection of positive and negative moods. By looking at the relationship between your health and your mood, the mood tracker can provide insight into how your condition and treatment may impact your emotional health, as well as effective coping strategies.

  1. Open the app on the device you are using and log in
  2. Click on the “Daily Moods” Health Tool
  3. Click on 1 of 22 moods that match how you are feeling
  4. Write a little something for your future self
  5. Click “Done”

You can only view you mood history if you have previously recorded a mood.

  3. Click on “History” (located on the top right)

With the Appointment Calendar you can note down all your doctor’s appointments to help you stay organized. You may also use this calendar to jot down any events related to your health.

  1. Open the app on the device you are using and log in
  2. Click on the “Appointment Calendar” Health Tool
  3. Type in what kind of appointment this is
  4. Click on the date button and pick the date that correlates with your appointment
  5. Click on the drop down menu to select the time of your appointment
  6. Type in any notes to remind yourself of the little things for your appointment
  7. Click “Add to Calendar”

You can only view your appointment calendar if you have previously added an appointment in your calendar.

At the moment, the app does not have notifications for appointments, but hold on to your seats because it is coming soon!

With the Symptom Tracker, you can monitor any number of symptoms you are experiencing as part of your health condition. You can customize your tool by setting it up with symptoms you typically encounter, and rate daily their severity and impact on your day. Once recorded, a graph will be populated to show trends over time.

  1. Open the app on the device you are using and log in
  2. Click on the “Symptom Tracker” Health Tool
  3. Click on “Add Symptom”
  4. Type in your symptom
  5. Click on the symptom from the drop down menu that matches
  6. Type in any notes to remind your future self
  7. Click on “Finish”

You can only remove a symptom if you have previously added a symptom.

  4. Click on “Remove”
  5. Click on “Save”

With the Journal, you can choose from a curated list of guided journaling topics to help you learn more about your condition, practice gratitude, recognize accomplishments, and achieve self-discovery. You may also choose to use it to journal about any other topics. Journaling is has been proven to be therapeutic for coping and dealing with recovery.

  1. Open the app on the device you are using and log in
  2. Click on the “My Journal” Health Tool
  3. Click on a topic you want to make an entry about
  4. Type in your entry
  5. Click on “Add to Journal”

You can only view your journal entries if you have previously made a journal entry.

Healthy Doses is a health tool to take your mind off the clinical and medical aspect of dealing with a health condition. With Healthy Doses, you can collect doses of healthy wisdom for your mind and spirit.

  1. Open the app on the device you are using and log in
  2. Click on “Healthy Doses” Health Tool
  3. Click on a type of dose you want to take (Gratitude, Optimism, Mindfulness)
  4. Click on “Thanks” to dismiss the dose or click Save it to save the dose to view in the future

You can only view your Health Doses if you have previously saved a healthy dose.

Using the other features from Health Storylines

Using the Circle of Support feature, you can invite others who are part of your care team – whether they are your family members, close friends, or a caregiver – to follow closely on your daily self-care activities and progress.

They can send you messages of support through the secure messaging feature on the app, and you can also choose to share with them certain parts of your story based on your use of the different Health Tools.

By default, a person added to your Circle of Support will not see any of your Health Tools activity until you have decided to share it with them.

  1. Click on”Circle of Support” tab located on the left side menu bar
  2. Click on”Add New Member”
  3. Type in the email address of the other person and click”Submit”

The person you invited will receive an email invitation to your Circle of Support.

Using the Groups feature on Circle of Support, you can organize your contacts into different groups and share different parts of your health story with them. The group system enables you to share some things with certain people, and not others.

To assign a person into a group after adding him/her into your Circle of Support:

  1. Click on Manage Groups
  2. Click on a group (Family/Friend) or add a new group by clicking on the Add Group button where then add a group name and click Save
  3. Click on people you want to include in the group you have selected and click Save

After inviting the people you wish to be part of your Circle of Support and putting them into the different groups,

  1. Click on a Health Tool that you wish to share
  2. Click on “Visibility” or “Who can view this story”
  3. Click on the group(s) that you want to share with and click”Save”

The My Storylines feature shows you your overall health story, based on your usage of the different Health Tools. By using the Health Tools regularly, you will see your health story unfold, giving you greater insight into your health. You can choose to show this page to your healthcare provider, which helps to facilitate your treatment management, or use it to inform yourself of how you are doing.
